*Assistance available to process insurance for the medication, or cash pricing. I will not bill insurance for my portion of the services. These expenses are eligible for HSA/FSA reimbursement. By request, I am happy to provide an invoice so you may request out of network reimbursement from your insurance company. Those who have Medicare/Medicaid may not legally request reimbursement.
**Depending on procedure, you may be offered an alternative neuromodulator such as Dysport or Daxxify
